Tsunade assumes her duties as the Fifth Hokage and begins treating the ninja injured during the recent crises. She awakens Kakashi Hatake and Sasuke from the effects of Itachi’s Tsukuyomi, but Lee’s crushed bones and damaged muscles require an operation with a significant risk of death. Although initially uncertain, Lee chooses surgery after Might Guy promises to share the consequences should the procedure fail.

Still humiliated by his defeat at Itachi’s hands, Sasuke challenges Naruto on the hospital roof. Their Chidori and Rasengan nearly collide before Kakashi intervenes, but the damage each technique causes convinces Sasuke that Naruto may have surpassed him. Kakashi later warns Sasuke that pursuing revenge will only destroy the bonds he still possesses.

The Sound Four ambush Sasuke that night and defeat him despite his Cursed Seal. They explain that remaining in Konoha will prevent him from gaining the strength needed to kill Itachi and invite him to seek power from Orochimaru. After reflecting on his priorities, Sasuke decides to leave. Sakura intercepts him at the village gate, confesses her love, and begs him to remain, but Sasuke thanks her before rendering her unconscious and departing with the Sound Four.
