Miyagi is introduced after an extended absence from school following his fight with Mitsui's group. His return initially adds another volatile personality to Shohoku, particularly because he and Sakuragi quickly misunderstand each other over their respective romantic frustrations. Once they recognize their similar circumstances, they become unexpectedly compatible.

His return gives Shohoku the natural point guard it needs. With Miyagi directing possession, Mitsui providing perimeter shooting, Rukawa creating offense, Sakuragi developing as a rebounder, and Takenori Akagi controlling the interior, Shohoku's eventual starting five takes shape.

In The First Slam Dunk, Miyagi becomes the principal viewpoint character and receives an expanded personal history not presented in the original manga at comparable depth. The film reveals his childhood in Okinawa, the enormous influence of his older brother Sota, Sota's death, and the grief carried by Ryouta, his sister Anna, and their mother Kaoru. This material reframes his confidence as something developed alongside fear, bereavement, and the pressure of following an admired older brother.
