Kaoru is caring but emotionally reserved, particularly when confronting grief. She tries to continue functioning as a parent while carrying pain that makes ordinary communication with Ryota increasingly difficult.
She can appear stern or distant, yet this distance is not indifference. Kaoru worries deeply about her children and reacts strongly when Ryota takes risks, especially when his behavior evokes memories she is struggling to process.
Her relationship with Ryota is therefore defined partly by things left unsaid. Both care about the other, but basketball and family memories create emotional pressure that they do not initially know how to discuss.

The First Slam Dunk uses Kaoru to explore Ryota's life outside Shohoku. Flashbacks to Okinawa show the Miyagi household before Ryota becomes the point guard seen in the main basketball story.
After the deaths of her husband and later Sota, Kaoru struggles to cope with the losses while raising Ryota and Anna. Sota's absence becomes particularly difficult because Ryota's basketball increasingly recalls his older brother.
The family eventually relocates from Okinawa to Kanagawa. The emotional distance between Kaoru and Ryota persists until the events surrounding the Sannoh game allow both to confront feelings that had remained largely unspoken.

Ryota Miyagi is Kaoru's younger son. Their love for one another is complicated by shared grief and difficulty discussing the family past.
Sota Miyagi is Kaoru's elder son and an important presence in the family's early life. His death leaves a lasting emotional wound for Kaoru and fundamentally changes her relationship with Ryota.
Anna Miyagi is Kaoru's daughter and Ryota's younger sister. Kaoru continues raising her alongside Ryota after the family undergoes its losses.
