Madoka believes happiness exists in relation to someone else's unhappiness. As a bullied child, he tried to accept his own misery on the assumption that his suffering allowed other people to be happy.
That reasoning eventually turned inward. Madoka realized that being unhappy itself made him happy, leading him to conclude that causing unhappiness in others should likewise bring them happiness. The result is a deeply warped approach to suffering in which torture can be rationalized as something beneficial.
He can present himself as shy and harmless, making the violence beneath that demeanor especially difficult for an unfamiliar opponent to anticipate.

Madoka is transformed into a Complete Forgery and receives the artificial Branch of Sin Jūshin Shinjū. Blood released from his fingers forms claw-like constructs capable of administering a specialized poison.
Rather than simply injuring a victim, his poison interferes with the senses. Madoka can create false sensations and make an opponent perceive forms of physical suffering that are not actually being inflicted, giving him an exceptionally effective method of psychological and sensory torture.
As a Complete Forgery, he also possesses the enhanced physical capabilities associated with the First Unit and is regarded as its strongest member.

Madoka's childhood is shaped by two major traumas: the death of his father during the Great Tokyo Earthquake and persistent bullying afterward. The philosophy he develops from those experiences follows him into Deadman Wonderland.
Tsunenaga Tamaki selects Madoka for the Forgery program, turning him into the leader and strongest member of the Complete Forgeries' First Unit. Madoka initially approaches Ganta as though he intends to help him before revealing that he wants to torture and kill him.
After learning how much Shiro matters to Ganta, Madoka redirects his torture toward her. Ganta and Shiro ultimately combine their efforts to defeat him.
