Sukuna enters the modern story when Yuuji swallows one of his fingers to gain enough cursed energy to fight a curse. Instead of being consumed completely, Yuuji becomes an unusually stable vessel capable of suppressing Sukuna.

Gojou tests the newly incarnated Sukuna immediately and demonstrates that the partial incarnation is not yet an immediate match for the strongest modern sorcerer.

Sukuna's influence increases as Yuuji consumes more fingers. During the detention-center mission, Sukuna takes control and demonstrates just how far above ordinary curses he stands by effortlessly destroying a finger bearer.

He uses the opportunity to manipulate Yuuji's body and later establishes the "Enchain" binding vow, securing one minute of future control under specific conditions while erasing Yuuji's memory of the agreement.

Sukuna also develops a conspicuous interest in Megumi Fushiguro and the Ten Shadows Technique. He refuses to explain the interest, but repeatedly protects or observes Megumi for reasons unrelated to ordinary affection.

During Shibuya, Sukuna is temporarily given control after Yuuji is force-fed multiple fingers. He fights Jougo and later confronts Mahoraga after the shikigami is summoned by Megumi.

Sukuna uses the Enchain vow to transfer himself from Yuuji into Megumi, taking advantage of Megumi's emotional collapse to turn him into a new vessel. He later uses the Ten Shadows Technique and Megumi's soul as part of his preparations for fighting Gojou.

After recovering his remaining power through nineteen fingers and the preserved remains of his former body, Sukuna fights Gojou in Shinjuku. He uses Mahoraga's adaptation as a model for extending Dismantle's target to space itself and defeats Gojou with the resulting world-cutting slash.

The surviving sorcerers then execute a prolonged succession of attacks designed to separate Sukuna from Megumi. Yuuji ultimately becomes the central opponent capable of striking the boundary between their souls.
