Sanosuke is brash, impulsive, fiercely loyal, and proud of his physical toughness. He enjoys fighting and often responds to complicated situations by proposing a direct confrontation. His apparently simple approach can hide considerable perceptiveness, particularly where political exploitation or personal cowardice is involved.
His strongest moral wound comes from the destruction of the Sekihō Army. Sanosuke despises the Meiji government's willingness to use idealistic people and later condemn them for political convenience. The "evil" symbol on his clothing reflects his decision to carry the stigma forced upon the people he admired.
Sanosuke becomes one of the most dependable members of Kenshin's circle precisely because he dislikes pretension. He argues with Kenshin when he thinks Kenshin is being self-destructive, mocks Yahiko without denying his courage, and shows loyalty through immediate action.

Sanosuke initially fights with a massive zanbatō, relying on extraordinary strength to swing a weapon designed more like an anti-cavalry blade than a conventional sword. Kenshin destroys it during their first confrontation, after which Sanosuke increasingly relies on bare-knuckle fighting.
His endurance is exceptional. Sanosuke can continue fighting after absorbing punishment that would incapacitate an ordinary person, and his approach often involves accepting damage to create an opening for a decisive counterattack.
During the Kyoto journey, he learns Futae no Kiwami from Anji Yūkyūzan, a technique that delivers two impacts almost simultaneously so that the second strike lands after the first has neutralized the target's resistance. Sanosuke adapts it primarily into punches capable of shattering rock and other hard materials.
Repeated overuse damages his right hand, forcing him to adjust how and when he deploys the technique. His later battles demonstrate greater tactical control rather than simple reliance on overwhelming force.

Sanosuke first challenges Kenshin as a hired fighter known as Zanza. Learning that Kenshin was once an Ishin Shishi initially intensifies his hostility because Sanosuke associates the revolutionaries with the betrayal of the Sekihō Army.
After Kenshin defeats him and forces him to reconsider his indiscriminate resentment, Sanosuke becomes a regular ally of the Kamiya dojo group. His personal history resurfaces through Katsuhiro Tsukioka, another Sekihō survivor who wants violent revenge against the government.
He follows Kenshin to Kyoto, learns Futae no Kiwami, and becomes one of the principal fighters in the confrontation with Shishio's organization.
The later manga reveals that Sanosuke's original family name is Higashidani. Returning to his birthplace brings him into contact with his father Kamishimoemon, sister Uki, and younger brother Ōta, who grew up after Sanosuke left home. These events reconnect the identity "Sagara" with his deliberate choice to carry Captain Sagara's name rather than his birth family's.

Sōzō Sagara was Sanosuke's captain in the Sekihō Army and the source of the surname he continues using. The government's execution and public disgrace of Sōzō becomes the defining trauma of Sanosuke's youth.
Kenshin Himura begins as Sanosuke's opponent and quickly becomes one of his closest friends. Sanosuke respects Kenshin's strength but is willing to challenge his decisions when necessary.
Kaoru Kamiya frequently scolds Sanosuke for his manners and freeloading, but the two share genuine trust.
Yahiko Myojin has an argumentative younger-brother relationship with Sanosuke. Sanosuke teases him constantly while quietly recognizing his growth.
Megumi Takani regularly treats Sanosuke's injuries. Their exchanges are abrasive, but Sanosuke is deeply concerned about her safety.
Anji Yūkyūzan teaches Sanosuke Futae no Kiwami despite belonging to Shishio's Ten Swords. Their eventual battle pits two men driven by anger at social injustice against one another.
