Maiko presents herself as a capable and professional WSG employee. Her public-relations position requires her to interact with people involved in a major international event, and she speaks both Japanese and English fluently.
She also believes strongly in fate and carries intense feelings concerning the FBI. Those feelings originate in the case against her father, Makoto Ishihara, whom she believes was wrongly accused of a series of crimes in the United States and abandoned by the authorities despite evidence supporting his innocence.
Her composed professional exterior conceals considerable determination. Maiko spends years holding the FBI responsible for what happened to her family and eventually channels that resentment into a carefully prepared revenge plan.

She understands the organization surrounding the World Sports Games and can move naturally among personnel connected to the event. This access becomes essential to the kidnapping plot, allowing her to operate near intended targets without initially attracting suspicion.
Maiko is also capable of preparing and executing technologically complicated sabotage. Her actions involving the Hyper Linear and associated facilities require advance planning and coordination with an accomplice who possesses specialized engineering knowledge.

Maiko works in public relations for WSG Japan during The Scarlet Bullet, when Tokyo is preparing both for the World Sports Games and the inaugural run of the vacuum-tube Hyper Linear. A sequence of kidnappings targeting figures associated with the event draws Conan Edogawa, Masumi Sera and the FBI into the investigation.
Conan eventually identifies Maiko as one of the people behind the incidents. Her actual identity is tied to Makoto Ishihara, the man blamed for a related series of kidnappings in Boston fifteen years earlier. Maiko believes her father was falsely convicted after the FBI disregarded the alibi she provided for him.
After her father died in prison and her mother subsequently died, Maiko returned to Japan and lived under the Shirahato name. She eventually targets Alan MacKenzie, who had been responsible for the FBI investigation connected to her father's case.
On the Hyper Linear, Maiko attempts to complete her revenge against Alan, but Conan and his allies expose the plan. The investigation then reveals that she has not been acting alone.

Makoto Ishihara is Maiko's father. His conviction and death in prison shape Maiko's life, and her belief that the FBI knowingly ignored evidence of his innocence becomes the central motivation behind her revenge.
