Kenzou Tenma

Tenma is compassionate, principled, hardworking, and unusually unwilling to rank one human life above another. His medical skill gives him opportunities to pursue status, but the central crisis of his early career forces him to decide whether a doctor's duty changes according to wealth or political importance.

He initially participates in Eisler Memorial's hierarchy more readily than he later admits. He benefits from Director Heinemann's favoritism, accepts Eva's expectations about their future, and allows senior administrators to redirect his surgical expertise toward prestigious patients. The death of a Turkish worker whom Tenma had originally been scheduled to treat forces him to recognize the moral cost of that arrangement.

Once Tenma commits to a principle, he can become stubborn to the point of self-destruction. He sacrifices sleep, food, safety, career prospects, and freedom rather than abandon someone he believes he can save. Even people who encounter him only briefly often remember him because he treats their lives as important regardless of status.

This compassion survives his pursuit of Johan, creating the central contradiction of his journey. Tenma becomes convinced that killing one person may be necessary to prevent many deaths, yet every instinct developed through a lifetime of medicine tells him that a life must be saved whenever possible.

His kindness is not naivety. Tenma becomes increasingly capable of recognizing manipulation, surviving criminal environments, using weapons, and dealing with violent people. His moral principles remain intact because he repeatedly chooses them after understanding the danger rather than because he remains sheltered from it.

Tenma is an elite neurosurgeon whose technical skill is recognized throughout the series. He performs extremely delicate procedures under pressure, saves patients other doctors consider hopeless, and can improvise medical treatment with limited equipment.

His medical knowledge extends beyond surgery. While traveling as a fugitive he treats gunshot wounds, trauma, infections, and other emergencies, frequently earning the trust of strangers through practical care before they know anything about his legal situation.

Tenma becomes competent with firearms after receiving training from Hugo Bernhardt. He learns handgun and rifle use because he believes confronting Johan may eventually require killing him. His marksmanship improves substantially, although his psychological resistance to taking a life remains the larger obstacle.

He is also physically resilient. Years spent moving across Germany while avoiding police surveillance require endurance, improvisation, and an ability to function under prolonged exhaustion. Tenma is not a professional fighter, but he becomes capable of surviving environments far removed from an operating room.

His greatest ability remains the trust created by his actions. Tenma's tendency to help people without expecting payment or loyalty produces a network of individuals willing to shelter, defend, inform, or assist him later. This social effect forms an important counterpoint to Johan, who also changes people's behavior but does so through manipulation and despair.

Tenma begins as Eisler Memorial's rising star, valued by Director Heinemann both for his surgical talent and as a prospective son-in-law. The arrangement begins to trouble him after hospital administrators redirect him away from ordinary patients whenever someone more politically valuable arrives.

When Johan is brought in with a gunshot wound to the head, Tenma is assigned to operate. The mayor of Düsseldorf then arrives, and Heinemann orders Tenma to abandon Johan. Tenma refuses, reasoning that the child arrived first and will die without him.

Johan survives while the mayor dies under another surgical team. Tenma is demoted, his research prospects disappear, and Eva ends their engagement. Shortly afterward, Heinemann and two senior physicians die suddenly.

Johan is eventually revealed as responsible for those hospital deaths, having interpreted Tenma's complaints about the administrators literally and removed the people who had damaged the doctor's career. Tenma becomes Chief of Surgery after their deaths without knowing why they occurred.

Nine years later, Tenma encounters Adolf Junkers, a criminal terrified of a "monster." Following him eventually brings Tenma face-to-face with the adult Johan and reveals that the boy he saved has become a serial killer. VIZ's official synopsis identifies this realization as the beginning of Tenma's desperate pursuit.

Tenma leaves his medical career and begins tracking Johan while Runge increasingly identifies him as the likely murderer. His journey takes him through former East German institutions, neo-Nazi organizations, criminal networks, Munich financial circles, Prague's communist-era experiments, and finally the history surrounding the Red Rose Mansion.

Along the way, Tenma repeatedly stops to treat strangers even when doing so risks capture. These episodes become central to the story because every life he saves challenges the premise that saving Johan was simply a mistake that should be corrected by killing him.

The pursuit eventually leads to Ruhenheim, where Johan orchestrates a massacre designed partly to erase the remaining people connected to his past and force Tenma toward a final decision. Tenma reaches him with multiple opportunities to shoot but remains trapped between his intention to stop Johan and the principles that made him a doctor.

Johan Liebert is the child Tenma saves and later the person he feels responsible for stopping. Johan Liebert repeatedly challenges Tenma's conviction that human lives possess equal value, while Tenma struggles with the belief that saving Johan may have caused every subsequent murder.

Anna Liebert becomes one of Tenma's closest allies in understanding Johan. Anna Liebert shares his desire to stop her brother but also shares his difficulty accepting murder as the solution, making their separate pursuits morally parallel.

Eva Heinemann is Tenma's former fiancée. Tenma initially plans a future with Eva Heinemann, but her abandonment after his demotion exposes how much their relationship depended on status. When she later attempts reconciliation, he refuses.

Udo Heinemann is Tenma's hospital director and intended father-in-law. Udo Heinemann promotes Tenma while he is useful to the hospital's prestige, then punishes him immediately after he disobeys an order involving the mayor.

Heinrich Runge pursues Tenma as his primary suspect for years. Heinrich Runge's confidence forces Tenma to live as a fugitive even while trying to prevent the crimes for which he is blamed.

Dieter is a young boy Tenma rescues from Hartmann's abuse. Tenma initially tries to place Dieter somewhere safe, but Dieter insists on traveling with him and becomes one of the clearest examples of Tenma creating hope where Johan's influence creates despair.

Julius Reichwein becomes an important ally after investigating deaths connected to Johan. Julius Reichwein supports Tenma's innocence and provides both psychological expertise and practical assistance.

Rudy Gillen is a criminal psychologist and former university acquaintance of Tenma. Rudy Gillen initially suspects Tenma may be the serial killer himself but gradually concludes that Johan is real and helps analyze the pattern behind his manipulation.

Wolfgang Grimmer meets Tenma through the investigation of former East German experiments. Both men are driven to protect children damaged by institutions, and Wolfgang Grimmer's experience with 511 Kinderheim gives Tenma access to a part of Johan's history he could not reconstruct through medicine alone.

Hugo Bernhardt teaches Tenma how to shoot. Hugo Bernhardt recognizes that the doctor's hands and mentality were built to save lives rather than take them, making the training as much a moral test as a technical one.
