Eva begins as Tenma's fiancée and the daughter of the hospital director whose approval is crucial to his career. When Tenma chooses to operate on a wounded child rather than abandon the scheduled patient for the city's mayor, Udo Heinemann punishes him professionally. Eva responds by ending their engagement, making clear that her commitment had been inseparable from the future Tenma's status promised.

After Udo Heinemann and two senior doctors are murdered, Tenma unexpectedly rises to Chief of Surgery because the positions above him have been emptied. Eva attempts to reconcile with him once his prestige returns, but he rejects her. That rejection becomes one of the central sources of her hostility toward him.

Nine years later, Eva has been divorced three times and lives largely from settlements while drinking heavily. She becomes involved with Inspector Heinrich Runge's investigation because she possesses information about Tenma and the hospital murders. Her resentment makes her willing to support the case against him even when she is not convinced he committed every crime attributed to him.

Eva eventually remembers seeing Johan on the night of one of the murders, making her testimony capable of undermining the accusation that Tenma is the killer. Roberto is subsequently ordered to eliminate her before she can become useful to Tenma's defense. VIZ's official Volume 7 description centers precisely on Eva's recovered memory and her value as a witness.

Her later storyline shifts away from revenge against Tenma through her association with Martin Reest. Martin is assigned to protect and control her but develops genuine concern for her, and his eventual fate forces Eva to confront another relationship lost partly because of the violent world surrounding Johan. The experience substantially changes the way she views Tenma and her own past choices.
