Anna appears first as the surviving twin connected to the shooting that brings Johan into Eisler Memorial Hospital. After the children's disappearance, the story jumps forward nine years and follows a young Heidelberg student who has grown up without a complete understanding of that night.

Her twentieth birthday becomes a turning point when Johan deliberately re-enters her life. Tenma, meanwhile, has discovered that the boy he saved is connected to a series of murders and begins searching for Johan's twin sister because she may hold information about his past. VIZ's official synopsis for Monster: The Perfect Edition, Vol. 1 identifies Tenma's search for the killer's twin sister as central to uncovering the "Monster."

Anna subsequently begins pursuing Johan herself. Her objective overlaps with Tenma's but is not identical: she wants to confront the person tied to the destruction of her childhood and recover the memories necessary to understand why their lives unfolded as they did.

The investigation takes her from Germany to Prague and into the history of the Three Frogs building, the Red Rose Mansion, and the Czech secret-police experiments surrounding the twins. VIZ's later volume descriptions explicitly connect Anna's memories to Prague and the Red Rose Mansion.

Anna eventually recovers the crucial memory that she—not Johan—was the child taken to the Red Rose Mansion and exposed to the massacre there. Johan had listened to her account afterward and absorbed the experience so completely that the twins' memories became entangled. This revelation changes how she understands both Johan's trauma and her own.
