He is proud of his ability and struggles intensely when circumstances threaten to separate him from the sport. Basketball is not simply an extracurricular activity for Ichiro; it is bound to his imagined future, his admiration for Rukawa, and the identity he has built as Tomigaoka's captain.
His interactions with Shohoku reveal a strong competitive instinct. He does not merely want sympathy or ceremonial participation. When given an opportunity to play, he wants genuine basketball, which helps explain why Rukawa's uncompromising attitude toward him ultimately carries more meaning than obvious pity would.

Ichiro's story is exclusive to the fourth original Slam Dunk anime film. His older sister Akane Mizusawa, a friend of Haruko Akagi, approaches Haruko about fulfilling her brother's wish to play with Rukawa. Toei places the story while Shohoku is preparing for its decisive match against Ryonan.
Ichiro has been diagnosed with articular tuberculosis affecting his right leg, threatening to end his playing career and destroying his plan to join Rukawa at Shohoku. His final request is to share a court with the senior he admires, prompting Coach Anzai to arrange an intra-squad game involving him.
Ichiro initially hopes to play on Rukawa's side. Instead, the two ultimately face one another, and Rukawa refuses to patronize his junior by playing halfheartedly. The game allows Ichiro to confront the possible end of his playing career through serious competition rather than pity.
Afterward, Rukawa gives Ichiro his number 11 Shohoku jersey. Ayako also suggests another way for him to remain involved with basketball by following her path as a team manager. Because Ichiro is a film-original character, these developments do not occur in the original manga continuity.

Kaede Rukawa is Ichiro's former Tomigaoka senior and basketball idol. Ichiro's plan is to follow him to Shohoku and pursue the national championship beside him. Akane Mizusawa is Ichiro's older sister. Concerned about her brother, she approaches Haruko Akagi in an effort to arrange the opportunity he desperately wants.
