Hisagomaru is timid, insecure, and excessively apologetic. He hides in the Seireitei’s sewers and flees when approached, but his isolation comes from loneliness rather than hostility.
He shares Hanatarō Yamada’s aversion to violence. He refuses to kill a living creature for food and becomes distressed when his scavenging harms Hanatarō’s chickens. Despite his fearfulness, he acts decisively when others are endangered.

Hisagomaru’s ordinary blade heals rather than injures anything it cuts. The damage removed from a patient fills gauges on both the sword and the spirit’s face with red light.
Once the gauge is full, Hisagomaru becomes Akeiro Hisagomaru. His chest opens to reveal an enormous cannon that releases all the accumulated damage as a concentrated red energy blast. This attack is powerful enough to destroy a large Tōjū after it overwhelms several Shinigami.
While hiding underground, Hisagomaru also constructs simple traps, including explosive tripwires and concealed floor panels that release sewer water.

After Muramasa forcibly materializes him, Hisagomaru repeatedly attempts to become a sword again. When this proves impossible, he concludes that Hanatarō has discarded him and retreats into the underground waterways.
A series of thefts brings Hanatarō, Ikkaku Madarame, Hōzukimaru, and Nanao Ise into the sewers. A tentacled Tōjū attacks and defeats the investigators, prompting Hisagomaru to reveal himself.
He heals the injured combatants, fills his gauge, and destroys the Tōjū with Akeiro Hisagomaru. Hanatarō assures him that he was never abandoned, although Hisagomaru promptly becomes lost while following the group out of the tunnels. He later returns permanently to sword form after the final Tōjū is defeated.

Hanatarō Yamada is Hisagomaru’s master. Hisagomaru remains devoted to him and is overjoyed to learn that Hanatarō did not deliberately cast him aside.
Hōzukimaru and Nanao Ise investigate the disturbances caused by Hisagomaru’s lonely scavenging. Ikkaku Madarame accompanies them.
Muramasa is responsible for Hisagomaru’s unwanted materialization.
