Hiluluk is exuberant, reckless and sincere. He overestimates his medical competence and reacts to failed treatments by fleeing angry patients, yet he never exploits illness for wealth or status.
His philosophy comes from an experience in the West Blue, where the sight of cherry blossoms seemingly cured his own fatal condition. Hiluluk consequently believes that a person dies only when forgotten and that hope can heal wounds that conventional treatment cannot reach.
He opposes Wapol’s monopoly over medical care and wants to cure Drum’s “sick heart.” His courage reaches its clearest expression when he walks knowingly into a trap because even the possibility of sick doctors needing help is enough to compel him.

Hiluluk has no established superhuman powers and is an unreliable medical practitioner. His homemade treatments can produce bizarre or harmful effects, and Chopper’s later competence comes principally from Kureha’s instruction.
He is nevertheless knowledgeable enough to conduct experiments and investigate rare ingredients. His long-term research produces a powder that causes Drum’s snow to appear like pink cherry blossoms, achieving the symbolic treatment he envisioned for the kingdom.
Hiluluk also possesses substantial endurance. He survives explosions from his own experiments and continues working despite suffering from a terminal illness.
His greatest practical ability is inspiring others. He gives Chopper a medical identity, teaches him the value of life and leaves behind a national symbol that helps transform Drum Kingdom after Wapol’s fall.

Hiluluk finds Chopper wounded in a blizzard. He removes his clothing to demonstrate that he carries no weapon, then treats the reindeer and gradually earns his trust.
After a year together, Hiluluk drives Chopper away because his own illness is approaching its end. Chopper misinterprets the rejection, learns of the illness and returns with an Amiudake mushroom he believes can cure him.
Hiluluk recognizes the mushroom as poisonous but consumes the medicine because Chopper risked his life to obtain it. Learning that Wapol’s twenty doctors are supposedly ill, he goes to the castle, discovers the report was a trap and drinks an explosive potion so that Chopper will not blame himself for the death.
His cherry-blossom research is completed after his death. When Luffy defeats Wapol, Dalton and Kureha use Hiluluk’s powder to fill the winter sky with pink blossoms as Chopper departs.

Tony Tony Chopper is Hiluluk’s adopted son and student. Hiluluk gives him his name, his hat and the conviction that a doctor can become a symbol of hope.
Dr. Kureha is Hiluluk’s fellow independent doctor and reluctant confidante. She considers him a dangerous amateur but understands his ideals, treats his illness and accepts responsibility for training Chopper after his death.
Wapol criminalizes independent medicine and creates the despair Hiluluk wants to cure. Hiluluk defies his rule even when doing so makes every house call dangerous.
Dalton witnesses Hiluluk’s final stand and is moved by his condemnation of Wapol’s rule. He later helps realize the kinder country Hiluluk wanted Drum to become.
