Youko is polite and considerate, including when rejecting Sakuragi. She does not ridicule him or deliberately humiliate him, simply explaining that her romantic interest lies elsewhere.
Her feelings for Oda are sincere, and she worries about his tendency to place excessive pressure on himself as a basketball player. This gives her a more substantial role in the theatrical story than the unnamed girl glimpsed at the beginning of the manga.
She also retains concern for Sakuragi despite having rejected him. Seeing him genuinely commit himself to basketball changes the context in which she understands the former delinquent she knew in junior high.

Youko is not presented as a major competitive basketball player. Her role centers on her relationships with Oda and Sakuragi rather than athletic performance.
She nevertheless understands the importance basketball holds for Oda and follows his progress closely. Her familiarity with Sakuragi from junior high also allows her to appreciate how dramatic his new commitment to the sport is.

Youko's rejection provides the immediate background to Slam Dunk's opening. Sakuragi enters Shohoku after being rejected for the fiftieth time, and the fact that Youko preferred a basketball player contributes to his initial declaration that he hates basketball.
The first theatrical film brings Youko back when Shohoku plays Takezono. She is now attending the same school as Oda, giving Sakuragi an awkward reunion with both the girl who rejected him and the basketball player she preferred.
The situation initially feeds Sakuragi's rivalry with Oda, but the game ultimately shifts attention away from romantic embarrassment and toward the seriousness with which both boys now approach basketball.

Hanamichi Sakuragi knew Youko at Wakou Junior High and confessed his feelings to her. She rejected him because she was interested in Oda.
Tatsumasa Oda is the basketball player Youko admires and the reason she turns down Sakuragi. In the theatrical-film storyline, she attends Takezono alongside him and worries about the pressure he places on himself.
