Hokeni presents herself as confident, flirtatious and deliberately seductive. She uses Yugi's nervousness around her to keep him distracted and establish control over their encounter.

Her behavior during the duel is considerably more calculating than that of an ordinary school nurse. She understands how to exploit Yugi's inexperience and emotional reactions, suggesting from the beginning that the confrontation has been carefully arranged.

This personality is ultimately part of the deception surrounding the nurse, who is not an independent opponent at all but a life-sized puppet being operated by Ridley Sheldon.

Hokeni challenges Yugi to Duel Monsters and demonstrates enough skill to defeat him. The encounter is unusual because Yugi is accustomed to games triggering the appearance of Dark Yugi when the situation becomes sufficiently serious, yet the nurse succeeds before he can reverse the match.

She defeats Yugi and takes his Neon Knight card. The school-nurse identity is subsequently exposed as a disguise: the woman is a puppet remotely controlled by Ridley Sheldon, the first of Kaiba's Four Game Masters.

The encounter establishes the threat posed by the Game Masters and leads toward Yugi's later rematch with Sheldon.

Yugi Mutou is summoned by Hokeni to the school infirmary and challenged to Duel Monsters. She defeats him as part of the plan to draw him into a confrontation with the Four Game Masters.

Ridley Sheldon has a hidden connection to Hokeni. The school nurse is actually a puppet operated by Ridley Sheldon rather than an independent person.

Seto Kaiba initiates the larger campaign responsible for the encounter, ordering his Four Game Masters to target Yugi Mutou.
