Personality
Elder is compassionate, determined and unusually committed to the humanitarian side of medicine. She treats impoverished patients despite knowing that doing so will provoke hostility from people who profit from expensive medical care.
Her unusual public presentation is partly a practical response to prejudice. Elder conceals that she is a woman because she expects patients in Meiji-era Japan to place less confidence in a female physician. Rather than abandon medicine because of that discrimination, she adapts so she can continue treating people.
Elder is also perceptive about Kenshin. Their conversations allow her to recognize that his physical scar and emotional burdens are connected, and she encourages him to find somewhere he can rest rather than continuing indefinitely as an isolated wanderer.

Elder is a highly capable physician. She diagnoses illnesses, prepares medicine and treats patients regardless of their ability to pay. Her knowledge is her principal ability; she is not portrayed as a martial artist.
She also displays composure in dangerous circumstances. Even after becoming entangled in Ishizu's efforts to remove her and Espiral Rotacion's pursuit of Kenshin, she continues prioritizing the welfare of patients.
When Ishizu attempts to release smallpox, Elder reacts quickly enough to prevent the contaminated material from spreading, averting a potentially devastating outbreak.

Elder meets Kenshin in Yokohama through Dankichi Asahiyama, the rickshaw driver who assists her. Kenshin sees firsthand how she treats a sick woman whom Ishizu would rather leave without care because she cannot pay.
Ishizu's hostility escalates until Espiral is hired to deal with Elder. Espiral, however, becomes increasingly interested in Kenshin, turning the conflict into a confrontation between two swordsmen shaped by histories of killing.
After the crisis is resolved, Elder prepares to leave Japan for America, where other patients await her. Before departing, she advises Kenshin that healing requires him to stop wandering eventually and find a place where both his body and mind can rest. Her words foreshadow the home he soon finds at the Kamiya Dojo.

Kenshin Himura protects Elder and becomes one of her friends during his time in Yokohama. Their conversations reveal similarities between two people who have chosen lives centered on helping rather than harming others.
Dankichi Asahiyama is Elder's rickshaw driver and loyal assistant. She previously treated his broken leg and later employed him so he could earn a living.
Deian Ishizu opposes Elder because her charitable treatment undermines his profitable medical practice.
Espiral Rotacion is hired to eliminate Elder. In the 2023 adaptation, Elder's example helps challenge Espiral's belief that inherited expectations must determine the course of a person's life.
